ASSUMPTION OF RISK: I understand that using gym facilities and equipment involves inherent risks of injury. Activities include but are not limited to weight training, cardiovascular exercise, group fitness classes, and use of locker rooms and common areas. RELEASE OF LIABILITY: In consideration of being permitted to use the facilities, I hereby release Peak Performance Gym, its owners, employees, trainers, and agents from any and all liability arising from my use of the gym, its equipment, or participation in any classes or programs. HEALTH DECLARATION: I confirm that I am physically fit and have no medical conditions that would prevent safe participation in exercise activities. I agree to consult a physician before beginning any new exercise program.
