PET CARE AGREEMENT: I authorize Happy Tails Pet Care to provide grooming, daycare, boarding, and/or other pet care services for my pet as requested. ASSUMPTION OF RISK: I understand that interactions between animals carry inherent risks including bites, scratches, illness, injury, or escape. I accept these risks and agree that Happy Tails Pet Care has taken reasonable precautions. HEALTH & VACCINATION: I confirm that my pet is current on all required vaccinations (rabies, distemper, bordetella) and is free from contagious diseases. I agree to provide proof of vaccination upon request. EMERGENCY CARE: In the event of a medical emergency, I authorize Happy Tails Pet Care to seek veterinary treatment for my pet at my expense.
