PARENTAL CONSENT: I, the undersigned parent or legal guardian, give permission for my child to participate in activities organized by Bright Horizons Youth Camp including outdoor recreation, sports, arts and crafts, and field trips. ASSUMPTION OF RISK: I understand that participation in youth activities involves inherent risks including minor injuries, exposure to weather, and physical exertion. I accept these risks on behalf of my child. MEDICAL AUTHORIZATION: In the event of an emergency where I cannot be reached, I authorize the staff to seek and consent to medical treatment for my child.
